Face Lift


The face, our most visible feature, conveys how and what we feel through expression and appearance. A loss of skin elasticity and firmness, sagging tissues, wrinkles, and sun-damage all comprise facial aging. These visible signs of aging can strongly influence how we feel about ourselves and how others see us. Individuals who are energetic and excited about life can appear tired or sad due to aging in their brow, eyelids, midface, jaw line, and neck. The loss of youthful contours in these areas is the result of many factors including heredity, gravity, stress, and environmental conditions and exposure.

If the signs of aging on your face bother you, an array of non-surgical and surgical treatments including laser and chemical skin resurfacing, injectables and fillers, contour threadlifts, microfat grafting, mini or MACS cheek and facelift, or a full "SMAS" facelift may be the right choice for you. Generally speaking, a facelift corrects sagging facial skin and muscles, removes excess fat, and restores firmness thereby creating a more youthful, rested, and vibrant appearance. Our aim is to achieve the most natural outcome with the least invasive treatment possible. The ultimate goal is to establish a more youthful look, without exaggeration or the extreme. In fact, a facial makeover should avoid the extreme to look natural and not "operated on".


Where do I begin?

A consultation with plastic surgeon Daniel Golshani, M.D. is the first step to learn how a facelift can improve an aging face and neck. Dr. Golshani's consultations are designed to fully educate potential patients. Consultations typically include a discussion of your goals, an evaluation of your current signs of aging, the options available to you for non-surgical and non-invasive or invasive facial rejuvenation, the likely outcomes and potential risks and complications associated with the procedure, and the course of treatment that Dr. Golshani recommends for you.


Preparing for surgery

Daniel Golshani, M.D., F.A.C.S. will carefully explain the facial procedure you will undergo. Prior to your procedure, you will be given specific instructions regarding pre-operative medical clearance,diagnostic testing, day of surgery procedures, postoperative aftercare, and follow-up instructions and appointment. In addition, you will be asked to sign consent forms to assure Dr. Golshani that you fully understand the procedure that you will undergo and any potential risks or complications related to your procedure. Smoking, to any degree can result in severe complications such as skin necrosis and must be avoided three weeks before and up to six weeks after facelift surgery. Use of aspirin or non-steroidal anti-inflammatory agents must also be avoided peri-operatively to prevent bleeding or seroma after surgery.


Procedure Available for Facial Rejuvenation

Dr. Golshani strongly believes in the multi-modality approach to facial rejuvenation. While surgery often provides the longest lasting and most natural outcome, it is not the panacea, and must be recommended in conjunction with other non-invasive treatments.

A good example is the presence of fine wrinkles and spots in the sun-damaged skin which do not benefit from surgery but from skin resurfacing such as Fraxel laser treatment.

The following is a list of modalities available for facial rejuvenation starting from the least invasive and progressing to the most surgically involved procedure. Most often these procedures are combined to fit each patient based on his or her individual needs:
  • Chemical Peels and Microdermabrasion
  • Botox®; Restylane®, and other fillers
  • Intense Pulse Light (IPL)
  • Fraxel Laser Skin Resurfacing
  • Radio-frequency Skin Tightening (Titan® or Thermage®)
  • Contour Threadlift ®
  • Autologous Micro Fat Graft Facial Augmentation
  • Mini facelift, Mini Cheeklift
  • Endoscopic Mid Face and Cheek Lift
  • Endoscopic and open Brow Lift
  • MACS Facelift
  • SMAS Facelift
  • Neck Lift with Corsette Platysmaplasty
  • Facial Bone Contouring and Augmentation (implants)


It is always wisest to look for the least invasive means to achieve an aesthetic outcome in the face, but when appropriately selected and applied, facial plastic surgery combined with adjunct treatments can result most visibly in a long-lasting, natural, and youthful look.


Following surgery

Once your procedure is completed, a dressing will gently be placed around your face to minimize swelling and bruising. A thin tube may be present to drain any excess fluid that may collect under the skin. Before being released, you and an accompanying family member, friend, or caregiver will be given specific instructions that may include:
  • How to care for the surgical sit
  • Medications to apply or take orally to facilitate the healing process and reduce the potential for infection
  • Specific concerns to look for at the surgical site or in overall health
  • When to follow up with Dr. Golshani


Patients undergoing Facelift surgery will be cared for in an overnight facility such as Serenity for optimal safety and patient comfort.


Progress and Healing

Initial healing will include swelling and some numbness and discomfort that can be controlled with medication. Your skin may look bruised or discolored for approximately seven to ten days following your procedure. Additionally, facial movements may be temporarily restricted. These are all common conditions.

Until the initial swelling subsides, Dr. Golshani will instruct you to keep your head elevated at all times. You will also be provided with specific postoperative medications such as pain medications, antibiotics, anti-bruising agents, and eye drops, and instructions including how to cleanse and care for your face and hair. Following all of the instructions carefully will be essential to the success of your outcome. A return to light, normal activity is usually possible within a few days of surgery.

Generally speaking, facelift incisions are hidden and usually well healed within a week. You will be ready to return to work and light activity sometime between 2 to 4 weeks following your facelift. Many patients use cosmetics to camouflage any bruising that remains. Please consult our clinical aesthetician for appropriate post-operative camouflage and lymphatic facial treatments as necessary.

Healing will continue for several more weeks as swelling resolves and incision lines continue to improve. You must practice diligent sun protection until the healing process is fully complete. Continue to follow Dr. Golshani's instructions and attend all of your follow-up visits as scheduled.


Results and Outlook

It may take several months for swelling to fully dissipate and up to 6 months for incision lines to mature. Life-long sun protection will help maintain your rejuvenated appearance by minimizing photo-aging or sun damage. In addition, a healthy lifestyle will help extend the results of your rejuvenated and more youthful appearance. Most patients find that the benefits of their initial facelift results are long lasting, even as they continue to age.
